Four Mile Creek Health — SMS Notification Terms and Conditions

Program Description

Four Mile Creek Health operates an internal SMS notification program to deliver automated on-call shift reminders to clinic staff members. These messages notify the designated employee when their on-call monitoring period begins.

Who Receives Messages

Only current employees of Four Mile Creek Health who have provided their cell phone number to the clinic administrator and consented to receive on-call notifications are enrolled in this program.

Consent

By providing your cell phone number to Four Mile Creek Health during employment onboarding, you agree to receive automated SMS notifications related to your on-call scheduling duties. Consent is not a condition of employment and may be withdrawn at any time.

Message Frequency

You will receive one SMS notification per scheduled on-call day, sent at 4:30 PM Mountain Time. Message frequency varies based on your on-call schedule.

Message and Data Rates

Message and data rates may apply. Contact your wireless provider for details about your plan.

Opt-Out

You may cancel SMS notifications at any time. Reply STOP to any message to unsubscribe. After we receive your STOP request, you will receive one final confirmation message and no further notifications will be sent. To re-enroll, contact the clinic administrator.

Liability

Four Mile Creek Health is not responsible for delayed or undelivered messages. Delivery of SMS messages is subject to effective transmission by your mobile carrier and is not guaranteed. Carriers are not liable for delayed or undelivered messages.

Changes to These Terms

Four Mile Creek Health reserves the right to modify these terms at any time. Continued receipt of SMS notifications following any change constitutes acceptance of the updated terms.

Governing Law

These terms are governed by the laws of the State of Colorado.

 

Verbal Consent Script used during employee onboarding:

"As part of your employment at Four Mile Creek Health, we would like to send you an automated text message reminder on days when you are assigned to on-call monitoring. You would receive one SMS per scheduled on-call day at 4:30 PM letting you know your monitoring period has begun. Message and data rates may apply. You can opt out at any time by replying STOP to any message. Would you like to provide your cell phone number to receive these reminders?"

Employee provides number verbally. Number is recorded in the clinic scheduling system by the administrator. Consent is documented in the employee's personnel record. See full SMS privacy policy at:
