Landrey Fagan, MD, MS, NCMP
Board Certified Family Physician and Owner
Hello! Thank you again for your interest in Four Mile Creek Health!

Since moving to Colorado in 2018, I have fallen in love with the people, the community, and the spirit of the Boulder area. I have aligned my goals to ensure we all thrive in this beautiful community we call home.

With the support of my husband, three children, and our beloved dog, Peanut, I founded Four Mile Creek Health with a dedication to your healthcare and well-being.

Throughout my career, I have specialized in primary care and maternal health. While this work has been immensely rewarding, the administrative challenges have often hindered my ability to provide comprehensive, individualized care.

As a board-certified MD in both Family Medicine and Preventive Medicine, I have continually collaborated with MDs, DOs, NDs, and NPs, appreciating the unique insights each brings to tailored healthcare.

My approach to optimizing your health begins with understanding your current situation and listening to your story. Building a strong, direct connection is essential to supporting you through all your healthcare needs. I am passionate about helping you live your best life and I am eager to share my experience and support you on this journey.
